All individuals who are interested in participating in the development of Science Mission Requirements (SMR) for Regional Class vessels are asked to complete this form.  For those individuals who plan to attend the meeting, this form will also serve as a meeting registration form.  The information provided will assist the steering committee in planning a productive meeting.  All inputs are considered preliminary and it is intended that interactions of the workshop will determine the final SMR recommendations.  A summary of the questionnaire results will be provided to the workshop participants as an input to their considerations.
